OCLC records just 5 holdings, with the listing having an undocumented note stating the first edition, of the same year, has an eratta leaf at rear. No Yale listing mentions such, American Imprints notes just the one entry & one has to wonder which came first- the blank leaf as here, or the erattum. In either case, a rare book on the subject.
